Medical Consent

During the session accident may occur when NEAT staff are required to provide emergency first aid, i.e. bumped head. Please provide consent for NEAT staff to administer emergency first aid as and when required. At the end of the session staff will verbally inform parents of any accident that may have occurred. Follow up emails will be sent to the parent/carer the evening off the session for their reference.

Walk home permission
